CDC: DSAT Guidance on CDC Import Permits for Swine-Originated Flu
  1. A CDC Import Permit is required for the importation into the United States of virus isolates and/or clinical samples associated with the swine-origin, novel influenza A (H1N1) virus outbreak.



  1. A CDC Import Permit is not required for the (1) transfer of those virus isolates and/or clinical samples associated with the swine-origin, novel influenza A (H1N1) virus outbreak, which originate in the United States; or (2) the further distribution of virus isolates and/or clinical samples associated with the swine-origin, novel influenza A (H1N1) virus outbreak that originally had been imported into the United States under CDC permit.
  2. Information for obtaining CDC Import Permits is as follows:
    • CDC Import Permit applications are available online at: http://www.cdc.gov/od/eaipp/importApplicationForms.htm
    • Completed applications may be submitted via e-mail (sent to importpermit@cdc.gov) or fax (404-718-2093).
    • Applicants with questions about import permits should call the Import Permit Line at 404-718-2077.
